In this review, we summarize available clinical, immunological and histopathological data on patients with MOG + CNS demyelinating disease. Using pathophysiologically meaningful cell-based assays, this humoral response is extremely rare in adult MS and absent in classical AQP-4 + NMO, sharply differentiating the evolving group from both established disorders. Recent investigations discovered that a subgroup of these AQP-4 – NMOSD patients produce an ab response against myelin oligodendrocyte glycoprotein (MOG), a molecule expressed on the outer lamella of the myelin sheath. Delineating these patients concomitantly revealed that not all patients presenting with clinically NMO-suggestive disease phenotype express AQP-4 ab, which created the pathogenetically undefined category of NMO spectrum disorders (NMOSD). Nowadays, AQP-4 + NMO is considered an autoimmune astrocytopathy, in which CNS demyelination occurs only as a consequence of a primary destruction of astrocytes. Indeed, the greatest success so far in deciphering the pathogenesis of a CNS demyelinating disorder resulted from the discovery of anti-aquaporin (AQP)-4 antibodies (ab), which allowed progressive delineation of neuromyelitis optica (NMO), formerly considered a variant of the most common CNS demyelinating disorder, multiple sclerosis (MS), as a distinct disease. To a great extent, this may reflect that the group of inflammatory CNS demyelinating disorders likely contains multiple pathogenetically distinct disease entities. Marignier R.Extensive research over the last decades basically failed to identify a common cause of noninfectious inflammatory central nervous system (CNS) demyelinating disease. Carlos attached a 45-page, 189-paragraph second amended complaint as an exhibit. On August 16, Carlos filed a document titled “Motion for Leave to File the Authorized Second Amended Complaint.” He explained this was in compliance with the June 18 order authorizing the amendment and that he was adding his wife and son as plaintiffs. It cautioned Carlos that he “need to look and see exactly what ha to prove under the ADA, and if it doesn’t fit, going to have a problem later.” Shortly after, the district court entered its order dismissing the complaint as a shotgun pleading with leave to file a second amended complaint on or before August 16. for a disabled person.” The court explained it would permit Carlos to file a second amended complaint and warned Carlos that, based on the USCA11 Case: 22-14133 4 Document: 20-1 Date Filed: Opinion of the Court Page: 4 of 12 22-14133 caselaw cited by the defendant, his complaint sounded like medical malpractice, not an ADA claim. The court also asked Carlos how he knew he had difficulty getting Angie an appointment because of his disability, rather than the hospital being busy, and Carlos said the defendant’s staff treated his son “like a normal person,” rather than giving him an earlier appointment as “a reasonable accommodation. At a status conference in June 2019, the district court stated it would dismiss the first amended complaint as a shotgun filing because Carlos had reincorporated his prior allegations in each count. The defendant renewed its motion to dismiss, arguing the first amended complaint was a shotgun pleading and failed to state a claim. Generally, Carlos was dissatisfied with the treatment Angie received during the October and November visits, and his complaint asserted counts for discrimination and retaliation under the ADA and RA, breach of contract, and promissory estoppel. Carlos’s 26-page, 168-paragraph first amended complaint described three encounters with the defendant: (1) a phone call on May 3, 2016, to set up a doctor’s appointment, (2) an emergency room visit on Octoand (3) the scheduled appointment on November 4, 2016. BACKGROUND Carlos, as the sole plaintiff, filed a pro se complaint in 2018, and in response to the defendant’s first motion to dismiss, he filed his first amended complaint in April 2019 with the district court’s permission. For ease of reference, we refer to Carlos and Angie Alonso by their first names. After careful review, we affirm the dismissal. They appeal from the district court’s dismissal of their pro se second amended complaint as a shotgun pleading and alternatively for failure to state a claim. Over the course of their interactions, they allege the defendant discriminated against them based on Angie’s disability in violation of Title II of the Americans with Disabilities Act (ADA), 42 U.S.C § 12131-12134, and Section 504 of the Rehabilitation Act of 1973 (RA), 29 U.S.C. Ubisoft hasn't put together a city like this since 2015's Assassin's Creed Syndicate-even that rendering of Victorian London was so spread out that its protagonists needed a wrist-mounted zipline gun to get around-but navigating Baghdad is so natural that you'd think they never took an eight-year break. Buildings are densely packed with highways of awnings, clotheslines, and support beams for Basim's crossing. It feels great to be back in an urban jungle, too. Assassin's Creed Mirage - Bazaar" Getting around Minus the oppressive occupation by "The Order of the Ancients" (what they call the Templars in AC lore before they adopted that name), Baghdad is the series' most lively city since AC Brotherhood's Rome. Where AC1 showed off its crowd tech by letting Altair shove poor people out of his way, Mirage demonstrates attention to detail in the way vendors set up their stalls for business, friends hold picnics in rooftop gardens, and passersby have full conversations in Arabic. The city's golden-crisp sunshine, bustling markets, and vibrant decoration reads like a deliberate counterexample to the studio's cold, dour depiction of 12th-century Israel from 2007. I'm glad it's not, though, because I'm already smitten with Mirage's Animus avatar, the cool-but-kind Basim (last seen in AC Valhalla), and Ubisoft's gorgeous rendering of 9th-century Baghdad. In the right light, Mirage could be mistaken for a next-gen remake of Assassin's Creed 1. Mirage goes back to the old style, where you have to steer your way across handholds Climbing is realistic again: The AC RPGs had a simplified version of climbing where you could grip onto basically anything.I like it as both a useful tool against guards at your elevation, but they won't instantly notice a standing Basim on a roof above them (a great touch, because who wants to crouch all the time?) There's a crouch button: Newer AC games have this, but most of the old ones didn't.Pickpocketing, too: Just about anyone can be pickpocketed, but there's a quick minigame to pull it off.Notoriety is back: My scrap with the harbormaster left me with a notoriety status that made guards more attentive, but I was able to tear down wanted posters to clear my name like it was 2009.Mirage feels like classic AC, but controls like Valhalla: you can run without holding a trigger, combat has the same Souls-like button scheme as Valhalla (light, heavy, dodge), and Basim can pivot his feet on a dime. It typically costs half the price of the Nest Bedding Easy Breather, and the company offers a 30-day no-questions-asked return policy. The Xtreme Comforts pillow has a medium loft, and you can mold or adjust the filling to make it comfortable for people with various body types and sleep positions (even some of our stomach-sleepers have really liked this pillow). However, in the past few years, we haven’t seen complaints from readers or staffers about a chemical smell, so we think the problem may have been resolved-we’ll retest in a future update to be sure. And when we tested four Xtreme Comforts pillows in 2019, half of them smelled so bad that we couldn’t get to sleep even after several weeks, the smell didn’t fade (the other half of the pillows were odorless). Over time, several staffers and readers have reported that this pillow has occasionally arrived with a notably strong chemical smell. We’ve been recommending the Xtreme Comforts Shredded Memory Foam Pillow since 2016, and it was a top pick in previous versions of this guide. Garnet Hill has a 90-day return policy (changed in 2022, from the previous lifetime guarantee) and a one-year warranty against defects. It’s the only down-pillow pick in this guide, and it is Responsible Down Certified (RDS). In our 2022 testing, several back- and side-sleepers (who in testing have typically preferred taller pillows) also liked the firm Garnet Hill. This pillow is easier to shape to your shoulders and neck than other down pillows we’ve tried. If you are partial to it, note that our stomach-sleepers in particular preferred the firm-density Garnet Hill Signature White Down Pillow, with its give and softness, to other down pillows we’ve tried over the past four years (with 11 sleep testers). In our years of testing, we’ve found that down has a very specific appeal. Like our other Sleep Number pick, the PlushComfort Pillow Ultimate, this pillow comes with a 100-day return policy by mail and includes a one-year warranty against defects. That’s because adjusting this pillow is tidier than adjusting our Easy Breather pick (which has loose clumps of foam that can be messy to mold). We believe the PlushComfort Pillow Ultimate is the best option for side- and back-sleepers who aren’t sure how much loft they really need. (Don’t confuse this pillow with another shredded-foam pillow we tried, the Sleep Number ComfortFit Pillow Classic, which isn’t adjustable.) It’s similar to the Sleep Number PlushComfort Pillow Ultimate, but this one uses a mix of shredded foam and down alternative, instead of just down alternative. And there are three sealed, removable layers inside, so this pillow is easier to customize than any others we tried. The Sleep Number ComfortFit Pillow Ultimate is one of the loftiest shredded-foam pillows we’ve tested. Sleep Number has recently changed its no-return policy to a 100-day return policy by mail, and the company also provides a one-year limited warranty on its pillows. Most of our testers (even some back- and side-sleepers, who generally prefer shredded foam) found a comfortable height and enjoyed the cushioned support of the PlushComfort Pillow Ultimate. But the PlushComfort Pillow Ultimate includes all three options in one pillow: It comes with three sealed, removable layers of fill to help you achieve the best fit for your body shape. Most pillows we’ve tried come in soft, medium, or firm densities. (“Down alternative” is a fluffy fill, usually polyester or rayon, that mimics the feel of down but costs much less.) The Sleep Number PlushComfort Pillow Ultimate is the only customizable down-alternative pillow we’ve ever tested, and it’s ideal for stomach-sleepers who have a hard time finding the right height. This pillow comes with a 30-day return policy and a two-year warranty. The Easy Breather comes in four sizes, including a side-sleeper version with a curved edge for your shoulder (which we haven’t yet tested). This can be a messy process, but it does let you achieve a precise fit. The pouch also has a zipper, so you can still add or remove shredded-foam filling to achieve your ideal loft. The Easy Breather was redesigned in 2021 with a softer, cooler cover, and the shredded foam is now encased in its own washable inner pouch (making it the first fully washable shredded-foam pillow we’ve tested). It’s one of the most comfortable shredded-foam pillows we’ve found, and there isn’t a persistent problem with smelly foam (which we’ve encountered with shredded-foam pillows from other companies). The Nest Bedding Easy Breather Pillow has been a consistent tester favorite for its firm, adjustable support. the day the supplier would have, but for an undue delay, issued an invoice in respect of the supply for that consideration or part and.
